Purpose/Overview
The purpose of the Community Program Evaluation is to examine a program within your community. This program
evaluation will include collecting and analyzing data about a program’s goals, activities, outcomes and evaluation
methods to determine program effectiveness.
Preparation
1. Read Schoon, Porta, & Schaffer (2019) Chapters 2, 7, & 9; Savage (2020) Chapter 5; and review Healthy People
(HP) 2030 Goals.
2. Complete week 4 discussion questions.
3. Arrange to interview a person with one of the programs you identified in week 4 (preferable lead person).
Instructions/Grading Criteria
In your paper include the following criteria:
1. Program: [1 point]
Name of program/initiative. Name of person interviewed, his/her contact information, and date of interview. Please
note: if you are unable to conduct an interview, indicate where you gathered information (i.e. website, brochure,
etc.).
2. Program Overview: [2 point]
Describe the program. Why and how was the program started? How was the need identified? How many years has it
been in existence?
3. Program Funding: [1 point]
Identify specific funding sources (global, national, state, and/or local) for the program/ initiative.
4. Program Objectives: [1 point]
Identify the program objectives.
5. HP2030: [1 point]
Identify one specific HP 2030 goal and objective which is addressed by this program.
6. Program Evaluation: [2 point]
How is the program evaluated for efficiency and effectiveness? Do you feel the program evaluation methods, are
adequate for evaluating the program outcomes? Why or why not? How else can the program be evaluated?
7. Personal Reflection: [3 point]
Were you previously aware of this program? Do you believe this program meets a community need and should be
continued? If not, why? If yes, how could the program be enhanced? Other than through this program, how else can
the needs for this targeted problem [that this program addresses] be met?
